Change Applications (IATF 16949)

When your organization falls under any of the followings, please fill out the form "IATF 16949 Application for Change in Registered Contents (PDF:1.0MB)" and submit it to your local JQA office.

When there is a change on your registered scope written in your certificate/appendix (Registered Organization Name, location or Scope of Registration).
When there is an organizational change (except the ordinary personnel transfers and an organizational change without any impact to the contents of certificate/appendix).
When there is a change of top management (including site representative) or a person whose position is equivalent to the management representative of the management system, or a change of those authorizations to other positions.
When there is a change in the contents of process map (changes of process function/contents, changes of department in charge, addition/deletion of processes, or changes of process sequence/interaction).
When there is a significant change in the operation details or in the number of personnel.

For the relocation of manufacturing site, please submit the change application to your local JQA office before the relocation. For other changes, please submit it within 90 days from the date of change. According to the application, JQA conducts the Change Audit.
If your organization failed to notify JQA of the change which is required by the certification agreement, and considered that the change may have an impact to the validity of your management system, JQA issues a category-A nonconformity (major nonconformity) to your organization and requests you to take necessary corrective action.

1. How to fill out this form

In the "Person to Contact" field, please fill in the name of the person who prepares this form.
Please fill in the details of the change in the column, or attach a document describing its details instead.
Please be noted that JQA may require more detailed information when the change is significant.

2. Change Audit

When JQA judged that the applied change may affect the validity of the management system and next audit is not scheduled within 6 months, JQA carries out a Special Audit to confirm this change.
When the change is only in the name of organization or location address description due to administrative reasons, JQA may conduct a document review instead of Change Audit.
Same as ordinary audits, JQA will check the readiness status of the organization whether we could go on to the Change Audit. Please be noted that JQA may conduct a 1st Stage Audit (same as Initial Certification Audit) when the applied change may have a significant impact to your organization's management system. A Change Audit may be conducted together with ordinary Surveillance/Renewal audit.
When the result of Change Audit or document review proceeding is appropriate, and is approved through JQA's certification process, the changed registration certificate will be issued.
Audit Man-days for the Change Audit will be determined depending on the criteria standard and the content of change.

When the addition of new manufacturing site comes along with the change, the "Application Form for JQA Management System Certification/Registration (IATF 16949)" shall be submitted to JQA. Certification agreement shall also be concluded by and between JQA and the new manufacturing site to be added.
When the addition of new manufacturing site or addition of customer(s) comes along with the change, IATF OEMs (including its subsidiaries, affiliated brands and joint ventures) Supplier Codes shall be filled out together with the submission of its evidence documents. For your reference, please see the following OEMs for which the report to IATF is mandatory.

4. When Involving Contact Person Change

If there is a change in your contact or billing information, please also fill out the form “Contact Information Change Request (PDF: 90KB)”, and send it to your local JQA office.
